Description

The Homecare Service Coordinator will support homecare office growth by ensuring a high-quality and consistent focus on each client’s needs while seeking opportunities for client referrals.

Essential Functions of the Job:

  • Generate new homecare clients and referral resources by completing the following:
    • Complete 5 face-to-face meetings per month within the community with at least 3 meetings directed towards private pay clients and 2 meetings directed towards waiver cases
    • Schedule and complete 3 events per quarter split between private pay clients and waiver cases
    • Conduct 50% telephonic client visits of patient census every six (6) months
    • Conduct 50% in-home client visits of patient census every six (6) months
    • Conduct 2 new client sign-ups per month with a minimum of 90% accuracy on all paperwork with at least 1 new private pay sign-up (24 contracts per year)
    • Complete a minimum of 50 external, outbound calls per day (250 calls per week)
  • Coordinate, organize, and maintain homecare cases while sustaining a retention rate of 85% for existing clients
  • Identify and fulfill clients’ needs by maintaining a ratio of 85% of staffed clients compared to authorized, unstaffed clients
  • Partner with clients’ service coordinator, MCO, and account representative to troubleshoot authorization issues and resolve past-due billing denials
  • Partner with Homecare Advocates and Staffing Coordinators to grow the Homecare Division Office
  • Complete, organize, and maintain documents, records, reports, and Participant Feedback forms with accuracy and efficiency
  • Maintain compliance with federal and state regulations
  • Maintain relevant and up-to-date knowledge of all state regulations regarding DNA’s homecare procedures
  • Work in an office environment on a full-time basis unless otherwise specified by the Executive Team

  • Customer service skills required
  • Travel required
  • Excellent oral and written communication skills
  • General knowledge and capability to operate office equipment
  • Meet or exceed all essential functions of the job, including but not limited to, work performance goals

Physical Demands:

  • Ability to remain in a stationary position for the duration of the workday
  • Ability to move about the office, bend, and reach
  • Ability to lift up to 25 pounds occasionally

Work Environment:

Generally, works in an office environment but may occasionally be required to perform job duties outside of the typical office setting if specified by the Executive Team. Employees will be required to travel to clients’ private homes and for off-site events.
